Please go to the location that you were advised to go to during your first visit (Yojiro Yonemori Clinic or Yonemori Hospital) and bring your patient registration card with you to your second and subsequent visits.

  • Health insurance card (not necessary if you have already visited and shown your insurance card in the current month)
  • Medication booklet

Appointments

If you did not make an appointment during your previous visit, you will need to call to make an appointment.
Some departments require appointments for return visits, so please check the information specific to each department.

Medical Examinations: Brief Outline

  • STEP 1
    Reception

    Please go to the location you were advised to go to during your first visit (Yojiro Yonemori Clinic or Yonemori Hospital) and submit your patient registration card to the reception desk.

    【Visitors to the outpatient rehabilitation Clinic】
    After you have finished registering your visit at the automatic reception machine for returning visitors at Yojiro Yonemori Clinic, please come to the rehabilitation room on the 3rd floor. There is an automatic reception machine for returning visitors located on the 1st and 3rd floor of Yojiro Yonemori Clinic.
    ※We ask for your cooperation in slowing the spread of CoVID-19 by measuring your body temperature, answering questions about your physical health, and sanitizing your hands.

  • STEP 2
    Medical questionnaire

    If your symptoms have changed since your last visit, or if you have not been for a checkup for an extended period of time, a member of our staff (medical clerk) will go through a medical questionnaire with you.
    You may be asked to wait in the waiting area until it is time for your medical examination.

  • STEP 3
    Medical examination, testing, and treatment

    You will have a medical consultation in a specific department.
    ※We will let you know which facility to go to for your next visit (Yojiro Yonemori Clinic or Yonemori Hospital) depending on your medical examination and test results.
    ※The order in which you receive your examination or testing may change depending on thestatus of testing or availability of tests.

  • STEP 4
    Payment

    Please pay for your visit at the automatic payment machine or at the payment desk.
